A strong exceptions process keeps delivery moving while keeping risk visible and time-bounded. What is being requested: the specific gate or policy to bypass.Why: the delivery impact if blocked and why alternatives are not viable.Risk assessment: what could happen and what would be impacted.Mitigations: compensating controls (feature flags, limited rollout, extra monitoring).Expiration: a date when the exception ends and must be renewed.Owner: who is accountable for resolving it. Exceptions must expire automatically and require renewal with evidence.Track exceptions as backlog items with severity and due dates.Review exceptions monthly and remove the top recurring friction points by improving tooling. If exceptions become the normal path, it means your controls are not aligned with how teams work. Fix the controls, not the teams.
